Absence Excuse Form: When your child returns from an absence, please be sure to send in an excuse note.
Building Use Form: If you or your organization would like to use a space within the District, please fill out this form and submit to the building's secretary.
Educational Trip Form: Submit this form to your child's school prior to any educational tour or trip that is not sponsored by the district. Without this form, the absence will be unexcused. If you have questions, please call us at 814-231-1170.
Volunteer Checklist: Should you wish to volunteer in your child's school, please fill out this form and return it to the main office. A new form is required each year.
